The Jacksonville North Pulaski School District has partnered with ARCare to offer another Immunization clinic to the students of Jacksonville Middle School on Wednesday, December 13, 2023 from 10 am to 3 pm on the campus of Jacksonville Middle School. Below is a link and a QR code that will take you to the paperwork you must fill out. By registering for this immunization clinic, you are consenting to your child receiving the immunizations ARCare determines your child is missing.The Deadline to register for this immunization clinic is Wednesday, November 29, 2023 in order for ARCare to order the needed immunizations for this clinic.
This clinic will be for childhood immunizations that are required in order for a scholar to attend school. Scholars who are missing immunizations should have received a letter via email, in the mail or in person informing the parent/guardian of the immunizations that they are missing. If you aren't sure if your scholar is missing any immunizations please contact Jacksonville Middle School and speak with the school nurse.
